Assessments for End Users

 

Assessments are tools used to track progress, status, or required screenings over time. They can take the form of:

  • Scales/Matrices (e.g., Crisis to Thriving matrix)
  • Pre/Post Tests
  • HUD Homeless Entry & Exit Status
  • Checklists or Logs for tracking processes

Assessments are added to work templates by administrators, and only those included will be visible in a staff person’s workflow.

Assessment Features

  • Each assessment has a name and is linked to one or more program types by an administrator.
  • Recent assessments are accessible from the Household or Participant summary snapshot on the dashboard.
  • Assessments can be taken from the Work Template or sent to clients via email or SMS.
  • Assessments may be multi-page and divided into sections with labels for clarity.

Steps to Create or Send an Assessment

Adding a New Assessment

  • Click the Green plus (+) button to create a new assessment.
  • Select Participants.
  • In the Select as Assessment Start Date field, enter the date the assessment was taken.
  • Set a Follow-Up Date, if desired.
  • Click Start Assessment to walk through the assessment with the client. 

Emailing an Assessment

  • Click the email option to send an assessment to a client.
  • Choose how to send the assessment.
    • If the participant has an email in the system, the assessment will be sent there.
    • If no email exists, you can enter one for the participant.
  • Once sent, an arrow icon appears. Click it to view the send history of the assessment.
