Create Non-Program Service Forms

 

SUMMARY:

Non-Program Service Forms are used to record services provided to households, participants, or businesses/entities that are not tied to a specific program enrollment. Examples include answering inquiries, tracking board training hours, and recording donations.

Key Considerations Before Creating a Form

  • Purpose of the form (e.g., donations, volunteer hours, or services)
  • Financial benefits attached
  • Specific service items to capture
  • Whether outcomes need to be recorded

Types of Non-Program Service Forms

  1. Donation – Tracks monetary or in-kind donations.
  2. NP Services – Captures services provided outside of program enrollment.
  3. Volunteer Time – Records volunteer hours.

Steps to Create a Non-Program Service Form

  1. Navigate to Admin → Non-Program Service Forms and select ADD NEW NON-PROGRAM SERVICE FORM.
  2. Complete the Basic Info page:
    • Name the form.
    • Select the form type (Donation, NP Services, or Volunteer Time).
    • Add a description (optional).
    • Click SAVE to proceed.

Additional Setup Steps

  1. Service Groups – Add service items to track within the form.
    1. Click the Service Groups tab.
    2. Click ADD NEW SERVICE GROUP to open the Non-Program Service Group Information page.
    3. Select a Service Group from the drop-down list.
    4. Define the Service Item Sorting preference.
    5. Choose specific Service Items from the available list.
    6. Click SAVE AND CLOSE to apply changes.
  2. Outcomes (Optional, for NP Services Only) – Select potential service outcomes.
    1. Click the Outcomes tab (only available for NP Services forms).
    2. Click ADD EXISTING OUTCOME to open the Non-Program Service Outcome page.
    3. In the Select Outcomes drop-down, choose the desired outcomes.
    4. Click SAVE AND CLOSE to apply selections.
  3. Fund Sources (Optional, for NP Services & Donation Forms) – Attach funding sources for expenditures or donations.
    1. Click the Fund Sources tab.
    2. Click ADD FUNDING SOURCE to open the Funding Source - Non-Program Service Forms page.
    3. Select a Funding Source and allotment from the drop-down list.
    4. Click SAVE AND ADD NEW to add multiple funding sources.
    5. Once all funding sources are added, click SAVE AND CLOSE.
  • NP Services: Fund sources track expenditures like program-enrolled services.
  • Donation Forms: Fund sources record which account the donation is added to but do not update the funding source automatically.
  1. Transactions (NP Services Only) – Tracks expenditures tied to funding sources.

Attaching the Form to a Work Template

  1. Navigate to Admin → Work Templates.
  2. Select a template and go to the Non-Program Service Forms page.
  3. Click ADD NON-PROGRAM SERVICE FORM and choose the form from the drop-down list.
  4. Click SAVE AND CLOSE.
