Emergency Contact Form

 

EmpowOR’s Supplemental Forms collect participant information required by programs, agencies, or funders. The Emergency Contact & Relationships form is used to record:

  • Contact name
  • Relationship to participants
  • Permissions granted
  • Email
  • Phone number
  • Associated participants

Steps to Add a New Contact:

  • Open the Emergency Contact & Relationships form.
  • Click the green plus sign to add a new contact.
  • Enter the contact’s details, including relationship and permissions.
  • Note: Different household members can have unique contacts with varying permissions.

Fields with Managed Lists:

  • Relationship (e.g., parent, neighbor, relative) → Managed List: Emergency Contact Relationship
  • Contact Permissions (e.g., pick up child, medical permissions) → Managed List: Emergency Contact Permissions
