House Information Form

EmpowOR’s Housing Information Form is a supplemental form used to collect detailed housing data beyond the participant's address. It is household-based and can be accessed from the Household Dashboard.

Fields & Descriptions

• Dwelling Type – Type of living quarters for the household (Managed in ADMIN menu → MANAGE LIST ITEMS).
• Owner/Landlord – Identifies who owns or manages the dwelling.
• Rent/Mortgage Amount – Monthly rent or mortgage payment.
• Effective Date – Date the participant moved into the dwelling.
• Arrearage Due – Total unpaid rent amount.
• Date Rent First Past Due – The first missed rent payment date, if applicable.
• # Bedrooms – Number of bedrooms in the residence.
• # Occupancy – Total number of people living in the household.
• Year Built – Year the dwelling was constructed (if unknown, an “estimated year” option is available).
