EmpowOR’s Supplemental Forms collect participant information required by a program, agency, or funder. The Insurance Form specifically records insurance details about the participant or their family.

Fields in the Insurance Form

  • Insurance Type – Specifies the type of insurance. Options come from the Managed Items List → Insurance Type.
  • Insurance Vendor – Records the name of the insurance provider. Options come from the Managed Items List → Insurance Vendors.
