SUMMARY:

Three Options for Tracking Referrals

  1. Presumptive Eligibility / Prompted Referral
    • Uses program eligibility rules to suggest referrals.
    • Can stop or warn for enrollment if eligibility criteria aren’t met.
    • Requires setup at the program level.
  2. Internal & External Manual Referrals
    • Internal: Assigned to a staff member or a general department.
    • External: Directed to an organization listed in the system.

      3.   A broad referral category can be used but lacks follow-up tracking.

Considerations When Choosing a Referral Method

  • When should referrals happen during the client process?
  • Do staff know where to refer clients (internal & external)?
  • Should staff be prompted with a referral list?
  • Are eligibility rules clear enough for automated referrals?
  • Should a universal intake process be established?

Using the Referrals Tab

  • Found in Enrollment Service Records and Non-Program Service Records.
  • Allows staff to record multiple referrals and send emails.
  • External referrals require organizations to be set up in Manage Organizations.
  • General staff can be prioritized using a symbol (e.g., *Name).

Manual Referrals & Case Notes

  • Entered in Service Delivery Forms or from the Home Dashboard widget.
  • Adding a Case Note Category (e.g., "Referral Notes") helps track referral reasoning.
  • Staff receiving referrals should check case notes before contacting clients.

Managing & Monitoring Referrals

  • Home Dashboard: Referral widget shows status and details of assigned/made referrals.
  • Manage Referrals: Accessible through Participants > Referrals for tracking.
  • Advanced Reporting:
    • Referrals Report: Tracks referral volume and household outcomes.
    • Referrals - 125 Poverty: Analyzes referrals based on poverty level.

Referrals by Outside Orgs: Tracks external referrals to the agency.
