Assessments are tools used to track progress, status, or required screenings over time. They can take the form of:
- Scales/Matrices (e.g., Crisis to Thriving matrix)
- Pre/Post Tests
- HUD Homeless Entry & Exit Status
- Checklists or Logs for tracking processes
Assessments are added to work templates by administrators, and only those included will be visible in a staff person’s workflow.
Assessment Features
- Each assessment has a name and is linked to one or more program types by an administrator.
- Recent assessments are accessible from the Household or Participant summary snapshot on the dashboard.
- Assessments can be taken from the Work Template or sent to clients via email or SMS.
- Assessments may be multi-page and divided into sections with labels for clarity.
