The empowOR Plans form is a case management tool that allows counselors and participants to collaboratively create and track goals through a structured family plan. Each agency customizes its list of goals and action steps by defining Goal Dimensions (broad categories) and Global Goals (specific objectives). These goals can be linked to agency outcomes and assessment questions for progress tracking.
Key Components:
- Goal Dimensions: General categories that group related goals (e.g., Housing, Financial Management, Employment).
- Global Goals: Specific objectives under each dimension (e.g., "Find and move into a bigger apartment" under Housing).
- Agency Outcomes: Goals can be linked to predefined agency outcomes, ensuring measurable progress tracking.
- Assessment Integration: Goals can be tied to assessment questions (e.g., Crisis to Thrive matrix) for real-time status updates.
Setup Prerequisites:
Before using the Plans form, an empowOR administrator must configure:
- Goal Progress Ratings (e.g., No Progress, Little Progress).
- Plan Progress Ratings (same rating scale as goals).
- Goal Dimensions (e.g., Housing, Employment).
- Global Goals (specific goals under each dimension).
Permissions:
Only System Administrators can configure Goal Dimensions and Global Goals.
Setup Steps:
- Create Managed List Items
- Define Goal Progress Ratings & Plan Progress Ratings
- Define Goal Dimensions
- Set Up Global Goals
- Add new goals under the correct dimension
- Assign goals to General, Career, or Agency Plans
- Optionally, link goals to assessment questions
- Assign goals to agency outcomes for tracking
- Save changes
