Client Inquiry Form Instructions
- Complete the Online Form – Required fields are marked with a gold star.
- Add Household Members – After entering the first household member’s details:
- Click “Add Another Household Member” if there are more members.
- Click “Finish and Ready to Submit” if you are the only member.
- Confirm Household Members – Review the list of all entered members.
- If someone is missing, click “Add Additional Household Members.”
- Provide Consent – Check the agency’s general release box (this is required).
- Submit the Form – Once consent is given, the Submit button appears. Click OK to finalize submission.
