The Disability Plans/Referrals forms collect required disability information for the Head Start/Early Head Start program.
Household Intake Process
- Basic Info Collection – Enter basic information for each participant.
- Household Creation – Create a household record and enter household-level basic info.
- Supplemental Forms – Complete necessary forms in the Work Template (varies by funder/agency).
- Child Enrollment – Assign the child to the appropriate Head Start program and classroom.
- Activation of Health & Disability Forms – These forms become available only after enrollment.
Steps to Enter Disability Plans/Referrals
Accessing the Form
- Click on the Disability Plans/Referrals form in the Work Template.
Plan Disability Info
- Click the green plus to add a new disability plan.
- Required Fields:
- IEP/IFSP/504 Plan: Select the appropriate plan.
- Initial Service Date: Enter the date or provide a reason if blank.
- Enrollment: Required field.
- Adding Disabilities:
- Click ADD NEW → Select a disability from the list → SAVE.
- Recording Plan Meetings:
- Click ADD A NEW MEETING → Enter the date, meeting title, and meeting notes.
Plan Referrals
- Click the green plus to add a new referral.
- Required Fields:
- Applies To: Select the child’s name.
- All Dates: Enter required dates.
- Referred To: Choose from Admin-managed options.
- Select Eligible, IEP/IFSP/504 Plan, Disability, and Health Event (Admin-managed list).
