EmpowOR’s Supplemental Forms collect participant information required by programs, agencies, or funders. The Emergency Contact & Relationships form is used to record:
- Contact name
- Relationship to participants
- Permissions granted
- Phone number
- Associated participants
Steps to Add a New Contact:
- Open the Emergency Contact & Relationships form.
- Click the green plus sign to add a new contact.
- Enter the contact’s details, including relationship and permissions.
- Note: Different household members can have unique contacts with varying permissions.
Fields with Managed Lists:
- Relationship (e.g., parent, neighbor, relative) → Managed List: Emergency Contact Relationship
- Contact Permissions (e.g., pick up child, medical permissions) → Managed List: Emergency Contact Permissions
